This module allows your Miva Merchant store to request additional information from customers during checkout. You can display up to five different text boxes, checkboxes, or drop-down menus to collect information such as special instructions for the order, a message for a greeting card, newsletter request, etc. The messages can be displayed on any one of the three checkout screens; or you can present them on a separate screen by using this module in conjunction with the Checkout This module (available separately).

The module has HTML templates that let you tailor the look of the message fields to match the rest of your site. You can make the messages optional or mandatory. The messages are displayed on all subsequent checkout screens, as well as the emails to customer and merchant, and is saved in a database that you can view in Admin. The information can also be included in Truxoft emails and UltraBatch reports; and it can be accessed by OpenUI tokens and other "4th-party" software. The module also provides tokens for use with the Viking Phone Order Manager and Other Page Templates modules.

For text boxes, you can specify each box's size, as well as the text prompt that appears above it, and the maximum and minimum length for the customer's entry. For checkboxes, you can specify the text prompt, the position of the check box (left or right of the prompt), and whether it defaults to checked or unchecked. You can also specify that a checkbox must be checked in order for the customer to proceed to the next screen. This is handy for creating an "I agree to your terms" checkbox. For drop-down menus, you can specify both the label that the customer sees and the value that is returned; you can also create "select-one" options that force the user to make a choice.

After the customer enters their information, it is displayed on each subsequent checkout screen, so that the customer gets instant confirmation of what they entered. The messages are also included in the notification emails to both the customer and merchant; and they are also saved in a database. The module's Order View screen allows you to view this database, so that you have a way to save and confirm a customer's messages in case any emails are lost.
