How do I prompt for the Customer's name not just e-mail address and how do I create a token for it?

The module has 5 tokens that you can use in the template that will display text fields to collect any data that you want (where xx is the field length):

%field1|xx|%
%field2|xx|%
...
%field5|xx|%

For example, to add a 40 character long field to collect the customer's name, you could add the following to your template:

Your name: %field1|40|%

You can then display the collected data in the email by using, in the email template, tokens of the form:

%field1%
%field2%
...
%field3%
